This year’s summit will focus on best practices in delivering value-based care within the LTC industry including perspectives on some of the most innovative models developed by PAC operators. We have selected respected thought leaders to speak on topics within this theme and will continue to make updates to the agenda as we identify additional speakers and finalize topics and activities.

2019 Speakers

BRIAN FULLER

CEO

Integrated Care Solutions

Brian is a nationally-recognized expert in bundled payments and post-acute care. He is one of a select few to have successfully led provider and convener organizations participating with the Centers for Medicare and Medicaid Services (CMS). He is also an expert on the impact and importance of post-acute care in new healthcare reform environments, as well as care integration and partnership development. In 2017, he was named one of healthcare’s rising stars by Becker’s Hospital Review, and has been a featured expert, speaker or author for the American Hospital Association, NPR, U.S. News & World Report, Hospital & Health Networks, McKnight’s, ADVANCE for Long-Term Care, AHCA and LeadingAge. Prior to becoming the CEO of Integrated Care Solutions, Brian was an independent consultant for Anne Tumlinson Innovations. He also served as Chief Strategy Officer at Great Lakes Caring, and in executive positions at naviHealth, Avalere and Brooks Health.

CORY RUTLEDGE

Cory Rutledge is a Principal with the health care group of CliftonLarsonAllen (CLA), specializing in a variety of strategic, financial, and operational services for health care clients across the care continuum. His interest in serving organizations delivering solutions to older adults has provided him the opportunity to serve as CLA’s national senior living leader where he is responsible for overseeing and guiding the strategic direction of CLA’s senior living practice. He has 17 years of experience in providing professional services to health care clients. In addition to utilizing his expertise to serve healthcare clients throughout the country, he is also a frequent speaker at state and national health care conferences. Cory holds
a bachelor’s degree in Accounting and Finance from
Bethel University.

ESTHER MOAS

Senior Director for Care Continuum

Mount Sinai Health System

As the Senior Director for Care Continuum at Mount Sinai Health System in New York City, Esther is responsible for the health system’s post-acute care strategy and operations. Esther leads a team of population health nurses and therapists that focus on quality, outcome and utilization for patients in the community. She spent 10 years in post-acute care operations as a Regional Director and an Administrator at Premier Home Health Care, Inc., a certified and licensed home care agency. Prior to joining Mount Sinai Health System, she was a Director at ArchCare, which includes five skilled nursing facilities, a certified and licensed home health agency, PACE program, a managed long-term Medicaid plan, and a Medicare Advantage plan. She is LEAN Six Sigma Green Belt Certified and a member of Sigma Theta Tau International. Esther received her nursing degree and Masters of Science from NYU, and is currently obtaining a PhD in Nursing Practice at Yale University.

JASON FEUERMAN

SVP Strategic Development & Managed Care

Genesis HealthCare

Jason H. Feuerman is responsible for leading strategic and value-based initiatives as well as managed care at Genesis HealthCare that provide innovative, effective solutions to the challenges presented in the constantly evolving healthcare industry. Mr. Feuerman also leads Genesis Physician Services which employs and contracts with over 500 physicians and nurse practitioners who provide services to Genesis residents and patients. Moreover, Mr. Feuerman leads the Company’s BPCI and MSSP programs. Prior to joining Genesis, Mr. Feuerman served as Senior Vice President and President, Public Sector and Health Plan Division of Value Options, Inc. Mr. Feuerman also served as President of Bravo Health, a subsidiary of Cigna, Inc. and as President of Senior Care Centers of America, a leading provider of adult day health services. He holds a Bachelor of Science in Finance and Economics from the University of Maryland.

Jess Dalton is a future-focused healthcare executive with expertise in cross-continuum operations, service line management, physician relations, business development, and innovative healthcare delivery models.  Jess began his career as an EMT and Emergency Department volunteer, and quickly advanced in education and acumen to Executive and C-Suite leadership positions spanning acute, ambulatory, and post-acute levels of care.  Jess has spent considerable time abroad, implementing sustainable public health systems and focusing philanthropic and academic research efforts to serve vulnerable populations.  Jess applies his high-energy and enthusiasm toward early adoption and implementation of innovative payment and care delivery models, continuum-based pathways, episodic efficiencies, and high-quality patient outcomes.

Marc Zimmet is the President and CEO of Zimmet Healthcare Services Group, a leading consultancy focused on the skilled nursing facility (SNF) industry that supports over 2,500 providers/related stakeholders nationwide. Mr. Zimmet is a sought-after speaker on the topics of SNF reimbursement, compliance and operations, and has presented hundreds of educational sessions on behalf of state and national associations including the American Health Care Association, LeadingAge, American College of Health Care Administrators, NJ Hospital Association, American Health Lawyers Association, American Jewish Aging Services, American Medical Directors Association, Healthcare Financial Managers Association and the American Association of Nurse Assessment Coordinators. He holds an MBA in Healthcare Administration and a Bachelor of Science in Accounting.

NISHA HAMMEL

Senior Director of Population Health Management

American Health Care Association

Nisha is the Senior Director of Population Health Management for the American Health Care Association,
the nation’s largest association of long-term and post-acute care providers. Nisha is a health care leader and strategic thinker with 17 years of post-acute care experience. She has served in varied roles including Senior Vice-President of Operations, Senior Vice President of Home Health and Hospice, Chief Compliance Officer and Senior Vice President of Strategy. Most recently, Nisha spearheaded the due diligence exploration of a post-acute, provider-owned Medicare Advantage Special Needs Plan and led the advocacy efforts for LeadingAge Ohio. Nisha has a B.A. in Psychology from the University of Notre Dame and an M.S.W. from the University of Michigan, Ann Arbor. She also holds a Specialist in Aging Certificate from the University of Michigan.

STEVE ALLEGRETTO, CPA, MPH

Vice President of Finance

Yale New Haven Health System

Steve is the Senior Vice President of Finance and Chief Financial Officer at LMH Healthcare and Vice President of Finance at Yale New Haven Health System, a $4.5B health system with over 24,000 employees located primarily in Connecticut. He has been at Yale New Haven Hospital/Yale New Haven Health for over 33 years, serving in a variety of financial and operational roles. He has had various teaching responsibilities at Yale School of Medicine’s Epidemiology and Public Health Program, Quinnipiac University, Sacred Heart University and currently is an adjunct professor at Fairfield University developing an undergraduate program focused on healthcare financial management linked to patient value. Steve is a certified public accountant with an undergraduate degree in economics from Fairfield University and a master’s degree in Public Health from Yale University.
